(including claim service) that you have applied for or been supplied

When applying for or purchasing insurance through Canadian
Farm Insurance Corporation's representative or otherwise dealing with
us, it is necessary for you to provide personal information. By providing
this information directly to us or through an insurance broker, you have
consented to certain uses of that information.

When applying for an insurance product through a Canadian
Farm Insurance Corporation representative you voluntarily supplied the
personal information we need to offer the products and services you can
expect from a Canadian Farm Insurance Corporation policy. When you become
a policyholder, your contract with us will contain this statement as

Your consent may be withdrawn at any time. The withdrawal
of such consent may mean that we may no longer provide you with insurance
coverage and services or that your coverage must be reassessed. If you

and we will explain the consequences of withdrawing your consent.

Canadian Farm Insurance Corporation is in compliance
with all privacy legislation in force in Canada and will remain in compliance
at all times. You should familiarize yourself with privacy legislation

is required to obtain insurance coverage and the meaning of personal
